In consideration of being allowed to participate in the activities, events and programs of National Domestic Workers Alliance (NDWA) and Care in Action, Inc. (CiA), both inside and outside of its buildings and to use its facilities, equipment and machinery, I do hereby waive, release and forever discharge NDWA and CiA and each of its officers, agents, employees, representatives, executors, affiliates and sister organizations and all others from any and all responsibilities or liability for injury resulting from my participation in any activities, events, programs or use of any equipment or machinery at the facilities, events, activities and programs.
I do hereby release all of those mentioned and any others acting upon their behalf from any responsibility or liability for injury to myself or my minor child(ren), including those caused by the negligent act or omission of any of those mentioned or others, acting on their behalf or in any way arising out of or connected with my participation in any activities or events of NDWA or CiA or the use of any equipment of NDWA or CiA or at any NDWA event or program.
